Instructions to Complete Forms
For electronic submission:
- Please download the registration forms.
- Complete the registration forms by using Adobe Acrobat Reader to type in the required information. If you don’t have Adobe Acrobat Reader, you can install it for free here.
- E-sign the form by clicking the signature and select the option to set up a new self signed digital ID/signature. After three quick steps, you will be able to sign the form.
- Email the signed forms to admin@golestankids.com.
To submit by mail or in person, please:
- Download the forms.
- Print, complete, and sign by hand.
- Submit to Golestan in person or by mail.
